Challenge:
Managers are unaware of standard analysis tools and aren’t prepared to truly determine the root cause of an incident or event in their labs. Managers are also not proactively looking for root causes of problems in their work areas.

Solution:

  • Begin with a common incident in the home to set a comfortable tone, and transition from what learners may already know or think. Then, show them how to apply this knowledge to the labs.
  • Use an animated, “comic-strip” style story with sound effects to bring the learner in.
  • Throw learners into a simulation of a major event and have them complete the steps using the provided tools to conduct a root cause analysis. Thus, enabling them to determine the root cause.

Solution:

Workshop participants have shown great success using the “5 Why’s” approach to root cause analysis upon completion.

This daylong workshop introduces managers to the tools and overall mindset of root cause analysis. Our Harlan Laboratories project manager was in complete agreement with us that a lecture-based, “Death by PowerPoint” approach to this workshop would not work! So, we added some creative storytelling and simulation-based case study work to really engage our audience and ensure learning objectives were met.

The entire afternoon is spent in a simulation where small groups of learners must rely on all the tools and techniques they’ve seen to conduct a full root cause analysis of a major event. They’re allowed to make mistakes, go down the wrong paths, and see the consequences of their actions. Since Harlan is facilitating this workshop globally, we developed a detailed facilitator guide, participant workbooks, and activity cards for their use, ensuring each workshop experience is consistent with the next.
